Setting our rescue dogs up for success with their new families is something we at EOPR take very seriously. As a part of EOPR's adoption agreement, you must commit to a form of training classes with your new dog. Do you agree to enroll your dog in obedience class, socialization class, lessons with a private trainer, OR an online training program?

EOPR reserves the right to refuse ANY applicant based on any of the information provided to us on this form or references. These questions may seem irrelevant but they allow us to properly match each of our dogs to the most suitable home and place the most suitable dog with your family. A fully completed questionnaire is required as the first step to the adoption process. Owning a dog is a privilege, not a right! Please help us get a glimpse into who you are and why you are interested in adopting one of our rescue dogs!
